I have seen that you sell belt buckle blanks.

But I can't work out how to attach them to a belt. Do you have to glue a belt around the hoop or something?

Yes, you have to sew or snap one end of the belt around the loop of the belt buckle.
Here is an example of belts that have snaps so you can change belt buckles:
